Only use this Form if you want to request exclusion from (i.e., opt-out) of the proposed settlement class. For more information on the proposed settlement, please review the Notice.
The deadline to file a Request for Exclusion is March 2, 2026. In order to file an opt-out online, you must use the Class Member Identification Number and PIN included on the Notice emailed or mailed to you.
If you do not have the Class Member Identification Number and PIN and believe that you are a member of the Settlement Class, contact the Settlement Administrator, RG/2 Claims Administration at 1 (866) 742-4955 or DeltaVPPA@rg2claims.com.
You may also print and mail the Opt-Out Form so that it is postmarked by March 2, 2026.
